Responsibilities:
- Act as a primary non-clinical point of contact for patients
- Conduct wellness check-ins, follow-ups, and patient engagement activities
- Provide basic health education, lifestyle guidance, and wellness coaching (non-diagnostic)
- Perform community or field-based coordination, including local follow-ups and support as needed
- Assist patients with scheduling, referrals, and navigating health or wellness services
- Collect patient information and escalate clinical concerns to the Registered Nurse
- Maintain accurate documentation and follow escalation and compliance protocols
- Background in health-related fields such as nutrition, allied health, public health, wellness, or community health
- Strong communication and interpersonal skills
- Willingness and ability to perform community or leg work when required
- Ability to follow clinical direction and escalation guidelines
- Comfortable using digital tools for documentation and communication
- Certification or training in health coaching, nutrition, wellness, or community health
- Experience working with barangay health programs, NGOs, or community outreach initiatives
- Multilingual skills (Filipino, English, or regional dialects)
